Trip Forms and Instructions

To prepare for your Deer Valley Experience, the following forms and information need to be completed and returned by the following dates:

Thursday, February 19, 2026:

• Deer Valley Online Registration 
• Adult and Student Health Form 
• Deer Valley Permission Agreement 
• Release and Waiver 

These forms can be returned to your child's science teacher, along with payment for the trips.

Friday, March 13, 2026:

• Clearances (sent to District Administration Building)
