Trip Forms and Instructions

To prepare for your Deer Valley Experience, the following forms and information need to be completed and returned by the following dates:

• Deer Valley Online Registration due by February 14, 2025
• Clearances (sent to District Administration Building) due by March 14, 2025
• Adult and Student Health Form due by March 14, 2025
• Deer Valley Permission Agreement due by March 14, 2025
• Release and Waiver Form due by March 14, 2025

These forms can be returned to your child's science teacher, along with payment for the trips.
