Camp Release & Waiver Form
- Boyce staff members who are attending without children should choose Myself option.
- Adults who are attending with their child(ren) should choose Parent/Guardian and Minors option.
- Adults who are not attending and need to sign the waiver for their attending child(ren) should choose Minors Only option.
